HTML DocAssist is a web-based application used by rehabilitation therapists to administer patient records and assessments directly from a browser. Multiple facilities at different locations can be selected from a list and individual patient records within a facility can be accessed.

     Perl/CGI was used to generate the forms and submit the data into a central Sybase SQLAnywhere database on a Windows NT platform.
